Gateway Wheelers - Image Consent Agreement
You grant permission to Gateway Wheelers of The Old Rectory, The Broadway, Houghton-le-Spring, DH4 4BB and The True Colours Trust, The Peak, 5 Wilton Road, London, SW1V 1AP to use and disclose your:
· image, physical likeness, and name (Images); and
· voice and sound (Sounds),
· without restriction in any media or format. You will let Gateway Wheelers and The True Colours Trust, know if you wish to withdraw this permission.
Your permission includes Gateway Wheelers and The True Colours Trust, using your Images or Sounds for any publicity or advertising in connection with its business or programmes and disclosing your Images or Sounds to other organisations (such as media and advertising agencies) for any purpose.
You have volunteered your Images and Sounds and agree that you will not receive or request any payment, royalty, or other form of consideration for your Images, Sounds or participation in any project of Gateway Wheelers and The True Colours Trust.
You agree not to appear on any current affairs program, television program, radio interviews, in any press interviews, or in any other publicity purporting to represent Gateway Wheelers and The True Colours Trust, nor to speak on behalf of Gateway Wheelers and The True Colours Trust, or in relation to a project of Gateway Wheelers and The True Colours Trust, unless Gateway Wheelers and The True Colours Trust, has agreed to that publicity in writing and in advance.
If you are Over 16 years old, by selecting “Yes” in our Membership Form; you consent to the above.
If the individual is under 16 years of age or is a vulnerable adult; their parent, legal guardian or authorised legal representative must agree with the below:
· I confirm that I am the parent, legal guardian or authorised legal representative of this individual and warrant that I have lawful authority to sign this consent form for and on their behalf.
· I have read, and understand, the terms and conditions and consent to the individual providing their Images and Sounds on these terms and conditions.
If you are the Parent, Guardian, or Legal Representative of a participant; by selecting “Yes” in our Membership Form, you consent to the above.
